Free and Reduced Forms
The consent for disclosure form that allows families who qualify for free or reduced lunches to also qualify for free or reduced books and technology fees will now be a part of online enrollment.
There is automatic eligibility (Direct Certification) for free Child Nutrition Program benefits to students who are already eligible to received Food Assistance (FA), Temporary Assistance to Families (TAF), or Foster Children. Letters will be mailed from Seaman Food Service to eligible households in July. Households receiving this Direct Certification letter do not need to fill out a paper application. Households receiving the letter for Direct Certification have the right to decline the benefits.
Special Dietary Needs Request Forms
Your student’s school will make meal modifications prescribed by a licensed medical authority that is authorized by Kansas state law to write medical prescriptions (MD, DO, PA, or APRN) to accommodate a disability. A prototype Medical Statement to Request School Meal Modification is below and can be used to request a meal modification.
IMPORTANT: For a student who does not have a disability, a request for meal modifications can be made and the school MAY choose to make substitutions. Any modification of this kind must meet the reimbursable meal pattern.
